• Please arrive 15 minutes prior to your appointment time to fill out paperwork or feel free to download the paperwork from our website and fill it out in advance.
• Bring a list of all current medications that you take.
• Bring your current insurance card as well as your specialist co-payment amount. This will be required before seeing the physician.
• If you have been referred to our office, please have that physician send to us any relevant medical information prior to your visit.
• If you do not speak English bring someone with you to interpret.
